Position Summary
The role of the Practice Transformation - Lead reports to the Director of Value Based Programs. This position is responsible for overseeing the logistical management of initiatives including assessment, process mapping, and coordination of resource deployment in conjunction with the primary care clinic management team.
Essential Position Responsibilities
- Partners with clinics to collect data and coordinate information technology needs
- Remain knowledgeable regarding current regulatory guidelines
- Develop and provide educational material to staff members and providers
- Collect data from insurance payors and Center for Medicare Services and present data, as needed
- Maintain follow-up schedule for routine calls with necessary stakeholders
- Serve as ACO Population Health Nurse
- Maintain program portals including reviewing current members and assisting with adding and deleting members, as needed
- Maintain an effective working relationship with all stakeholders
Qualifications
- Education: Must be a graduate of an accredited nursing program
- Licensure and Certifications: Registered Nurse license and CPR, required. Valid driver’s license and auto liability insurance, required.
- Experience: Prior experience working within healthcare insurance industry with specific experience in PCMH and other practice transformation models preferred.
