Position Summary

The role of the Licensed Social Worker reports to the Clinic Manager in partnership with the Clinic Physician. This position is responsible for assisting patients and family members through case management services. This position serves as an integral member of the interdisciplinary team while ensuring all patient healthcare needs are met.

Essential Position Responsibilities

  • Evaluate patients to determine need for counseling and case management services
  • Contact and follow up with patients and family members to assist with psychosocial issues that are seen in a medical and integrative health services practice setting
  • Address patient concerns and goals while maintaining constant communication with the patient as well as a case manager while ensuring strict confidentiality
  • Work closely with interdisciplinary care team to develop care plan for patients
  • Research and refer patients to community resources
  • Create and implement various social, counseling, integrative health, and other programs, lectures, and community events as they relate to the integrative health services program

Qualifications

  • Education: Must be a graduate of an accredited Bachelor or Master of Social Work program
  • Licensure and Certifications: Arkansas Licensed Social Worker (LSW) or Arkansas Licensed Master Social Worker (LMSW), required.
  • Experience: Previous experience and knowledge of community resources and case management, preferred. Previous experience with family systems therapy, preferred.

Work Environment: This position will spend 60% of time sitting while performing work in a standard office environment and 40% of time standing and/or walking while pushing, pulling, lifting, and/or carrying up to 50 lbs.This position serves a culturally and linguistically diverse patient population.
