Position Summary

The role of the Social Worker reports to the Director of Case Management and Social Services. This position is responsible for assisting patients and their families with personal and environmental difficulties affecting their illnesses and subsequent medical care. This position is responsible for identifying and analyzing the social and emotional factors underlying client illness. This position will collaborate with internal and external clients to arrange post-discharge care needs.

Essential Position Responsibilities

  • Identify and evaluate social and/or physical factors which may either impede full recovery from illness/surgery or prevent the patient from maintaining a state of well-being
  • Communicate these factors to the healthcare team and performs interventions, as indicated.
  • Assists patient and their families with procurement of community resources, as needed
  • Assist with the overall discharge planning process with the patient and interdisciplinary team.

Qualifications

  • Education: Must be a graduate of an accredited masters of social work program
  • Licensure and Certifications: Licensed Master Social Worker (LMSW), required.
  • Experience: Previous experience with social work standards of practice and ethics, required. Minimum 1 year previous experience in an acute care setting, required.

Work Environment: This position will 70% of time sitting while performing work in a standard office environment. This position will spend 30% of time standing and/or walking while pushing, pulling, lifting and/or carrying up to 50 lbs. This position will serve a culturally and linguistically diverse patient population. This position may be exposed to communicable diseases and bodily fluids.
