Position Summary

The role of the Social Worker reports to the Director, RN Clinical Services and Regulatory Affairs. This position is responsible for assisting patients and their families with personal and environmental difficulties affecting their illnesses and subsequent medical care.

Essential Position Responsibilities

  • Maintain effective working relationships with internal and external clients
  • Identify and analyze social and emotional factors underlying patient illness
  • Collaborate with interdisciplinary team to perform interventions, as needed
  • Assist patient and family with procurement of resources and applicable educational material
  • Assist with discharge planning process

Qualifications

  • Education: Must be a graduate of an accredited Master of Social Work program
  • Licensure and Certifications: LMSW, required.
  • Experience: Previous experience as a social worker in an acute care setting, required.

Work Environment: This position will spend 50 % of time sitting while performing work in a standard office environment and 50% of time standing and/or walking while pushing, pulling, lifting, and/or carrying up to 50 lbs. This position will serve a culturally and linguistically diverse patient population. This position may be exposed to communicable diseases and bodily fluids.
