Description

POSITION SUMMARY

Under the supervision of the Director and under the direction of primary care, Health Care Coordinator is responsible for conducting nursing assessments of participants and their goals and works with the Interdisciplinary Team to develop the Plan of Care. Coordinate, provide and monitor on going nursing care / treatment as required. As a member of the interdisciplinary team the RN works with participants to increase independence and community integration through a model of self-determination which maximizes physical, mental and functional health. Collaborates with Quality Coordinator and Medical Director regarding quality improvement initiatives and providing supporting data collection for analysis. This position functions in both office and clinical settings, with exposure to communicable disease and body fluids. Majority of time is spent sitting (up to 60% of work time) with frequent standing and walking of short distances required (up to 40% of work time.) This position is frequently required to lift and carry objects weighing up to 10 lb. (up to 20% of work time.) Frequent local travel to other facilities and private homes is required.

LICENSES/CERTIFICATIONS REQUIRED

Unencumbered Arkansas RN License, CPR, AR DL, Auto Liability Insurance

POSITION REQUIREMENTS

Degree/diploma in nursing and current State of Arkansas Registered Nurse license and CPR certification required. Bachelor's Degree in Nursing preferred. A minimum of one year of personal, clinical/internship, and/or employment experience providing care or services for a frail or elderly population required. Experience in a community-based or long-term care setting involving the elderly preferred. Two years of community nursing preferred. All staff must be deemed free of communicable disease and stay current with required immunizations.
