Description

Position Summary

The role of the RN - Neuro Services Navigator reports to the Director of Nursing - Neurosurgical Services. This position is responsible for coordinating patient care across the continuum as it relates to patients with a primary neurological or spinal diagnosis. The position works closely with Stroke and Spine program coordinators to ensure program goals are being met and patient outcomes are supported. This position partners with other key stakeholders across the organization.

Essential Position Responsibilities

  • Assist patients and families in navigating care throughout the continuum of care
  • Provide education and support to patients and families
  • Assist with scheduling follow-up appointments
  • Collaborate with the members of the patient’s care team, the patient, and the patient’s family to ensure the goals of care are being met
  • Collaborate with clinics to assist in development of seamless discharge to post-discharge processes
  • Provide post-discharge support as needed
  • Coordinate care and communication between patient and/or family and care team
  • Perform follow-up phone calls for discharged patients
  • Perform data collection, analysis, and presentation
  • Collaborate with clinicians and program leaders to review and revise or develop patient education materials

Qualifications

  • Education: Graduate of an accredited nursing program
  • Licensure and Certifications: Registered Nurse license and CPR, required.
  • Experience: Minimum 2 years of clinical experience in acute care or case management, required. Previous experience neuro population, preferred.

Work Environment: This position will spend up to 80% sitting while performing work in a standard office environment and 20% of the time standing and/or walking short distances while pushing, pulling, lifting and/or carrying up to 50 lbs. This position may have exposure to communicable disease, bodily fluids, and radiation. This position serves a culturally and linguistically diverse patient population.
