Description

Position Summary
The role of the Certified Nurse Assistant reports to the Nurse Supervisor. This position is responsible for providing direct patient care while performing at home visits and completing all regulatory requirements.

Essential Position Responsibilities
• Assess and chart observations of the patient’s condition at each visit
• Complete evaluation tasks, including reviewing vital signs
• Complete written reports on patients in a timely manner
• Provide thorough education for patients and families on proper home health care procedures and strategies
• Coordinate care with members of the patient’s care team
• Recommend resources that may improve the quality of life for the patient and assist patient in maintaining their independence as is relates to CNA standards and responsibilities
• Assist patients with personal hygiene and basic care needs in a timely and safe manner
• Observe patients for emotional, behavioral, and/or physical changes and report changes to appropriate case manager


Qualifications
Education: High school diploma or GED and Certified Nursing Assistant program, required.
Licensure and Certifications: Certified nursing assistant and CPR, required. Valid driver’s license and auto liability insurance, required. Fingerprints every 5 years, required.
Experience: Minimum of one year of CNA experience, preferred.


Work Environment: This position will spend 60% of time standing and/or walking short distances while pushing, pulling, lifting, and/or carrying up to 50 lbs. This position will spend 40% of time sitting while performing work in a standard office environment. Frequent local travel required. This position has exposure to communicable disease, body fluids, and radiation.
