Responsibilities

  • Review Value Based Care Program performance data for attributed lives by program and create action plans that support population health management and the organizational goals.
  • Evaluate payer and internal reports to identify provider and office trends and opportunities.
  • Coordinate and facilitate monthly meetings with providers and staff to review current topics, metrics, and gap performance and opportunities.
  • Conduct patient outreach in accordance with HIPAA and patient privacy guidelines to support quality-related programs and ensure target goals are achieved.
  • Work closely with other Population Health Coordinators and Population Health support department to achieve organizational goals.
  • Responsible for working with patient and a patient’s care team to coordinate change readiness, needs assessment, and develop an individualized treatment care plan.
  • Collaborate with the patient and a patient’s care team to define and assist in achieving maximum levels of wellness and independence.
  • Assess barriers and or social determinates of health when a patient has not met treatment goals and is not following plan of care.

Qualifications

  • Graduation certificate from accredited LPN / RN School of Nursing
  • 12 months related experience and/or training
  • LPN or RN active license
  • Prior Family Medicine experience preferred, but not required
