What You’ll Do

Plan, organize and implement social work to patients and families. Plan for discharge and aftercare. Maintain contact with referring agencies and related people, and with community resources used for referral.

Requirements:

• Masters of Arts in Social Work or related field

• Preferred minimum 2 years experience
