Job Summary

Community Clinic is seeking a Behavioral Health professional to serve as a Behavioral Health Consultant (BHC) at our Primary Care and Specialty clinic in Rogers, AR. The BHC operates as a core member of the primary care team, providing targeted, evidence-based interventions for behavioral health concerns within a Primary Care Behavioral Health (PCBH) model. The BHC works with the Primary Care Provider (PCP), helping to assess, manage, and treat patients presenting with medical and behavioral health concerns. The goal is to treat physical and behavioral health as one unified concept of overall health. The BHC also works in our Medication Assisted Treatment (MAT) program to provide behavioral health support while patients are in the program. This is a full-time, exempt, W-2 position. The schedule for this position will be in-person, Monday-Friday 8:00a-5:00p.

Key Responsibilities

  • Work as an embedded member of the primary care team, contributing to holistic patient care.
  • Conduct assessments for mental health or behavioral concerns referred by primary care providers.
  • Deliver targeted interventions around problem-solving, skill-building, and behavioral activation.
  • Accept same-day referrals from PCPs and conduct immediate interventions when possible.
  • Provide immediate support for patients experiencing acute distress or suicidal ideation, triaging appropriately
  • Work closely with PCPs to provide recommendations regarding behavioral health interventions.
  • Assist the psychiatric providers with bio-psycho-social assessment psych intake sessions.
  • MAT program: Provide behavioral health support to patients in the program and work closely with the psychiatric providers in managing the care of the patient.
  • Assist in managing behavioral aspects of chronic conditions such as diabetes, hypertension, chronic pain, and insomnia.
  • Support lifestyle interventions for weight loss, smoking cessation, medication adherence, substance use reduction, and stress management.
  • Assist in designing clinical pathways for conditions such as depression, anxiety, chronic pain, and substance use.
  • Connect patients to community resources and specialty mental health providers when needed.
  • Maintain concise, problem-focused documentation in the electronic health record.
  • Participate in tracking patient outcomes and improving integrated behavioral health services.
  • Attend care team meetings and engage in ongoing professional development.

Key Responsibilities

  • Behavioral Health Expertise: Ability to use therapeutic modalities that match a primary care environment (such as behavior activation, solution focused brief therapy, value-based interventions such as ACT/FACT, motivational interviewing, etc.)
  • Medical Knowledge: Familiarity with common medical conditions and psychotropic medications.
  • Team Collaboration: Strong ability to function in a fast-paced primary care environment.
  • Communication Skills: Excellent ability to consult and communicate with medical professionals and other team members.
  • Flexibility & Efficiency: Ability to conduct high-volume, targeted patient encounters while balancing team collaboration and documentation.
  • Constant Learner: Engages in professional development activities to develop skills useful for your role as a BHC.

Required Qualifications

  • Master’s Degree in Social Work or Counseling required
  • LCSW or LPC required
  • Bilingual Speaking ability (English & Spanish) is required
  • Experience in mental health field is required
