Job Summary
Community Clinic is seeking a Certified/Registered Medical Assistant to work as a Care Coordinator for our Siloam Springs Medical Center. This position works with the multidisciplinary team to promote patient engagement and health promotion throughout the care continuum by facilitating timely, appropriate health care delivery resulting in improved patient outcomes. This is a full-time, hourly position. Schedule is M-F with no evenings or weekends.
Key Responsibilities
- Provides care coordination review, planning and follow-up services to assure patients progress through the continuum of care.
- Manages care transitions after ER and inpatient hospital discharge, to facilitate timely engagement with Primary Care Provider.
- Facilitates preventive wellness services and improvement in patient health outcomes, utilizing population health tools and reports.
- Provides patients with information on health promotion, chronic disease management, healthcare services and resources to support their needs and goals.
- Coordinates the integration of social service, behavioral health, and case management into patient care.
- Collaborates with clinical and other case management staff in the development and execution of the plan of care and achievements of goals.
Qualifications
- Certified/Registered Medical Assistant required
- 1+ years of experience in a clinic or ambulatory care setting required
- Bilingual English/Spanish is a plus
- Current Basic Life Support certificate required
