This is a weekend position that will work 4 x 10 hour days, or 3 x 12 hour days. This position will assist with planning, organizing, directing, coordinating, and evaluating bed assignments in 2 hospice homes. Participates in delivery of skilled care when needed and assist with continuity of care coordination.
3 or more years of progressively experience in acute care nursing. Demonstrated leadership experience as charge nurse or in other capacity. Experience managing staff on a shift basis and recent experience in a skilled facility or as a case manager of hospice patients required.
- Facilitates and oversees safe and smooth patient transfers in and out of IPU.
- Manage shift staffing. Assures continuity of care through effective scheduling of staff.
- Serves as a resource for all IPU needs. Assists with facilitating solutions and processes.
- Collaborates with outpatient service line to ensure continuum of care services for patients.
- Articulates needs of patients transferring between levels of care to staff and promotes continuity of care.
- Assure compliance with regulations and contracts.
- Organize, prioritize, and delegate to meet routine daily needs, and in crisis.
- Utilize acuity and intensity tool to manage nurse staffing levels for patient census.
- Maintain knowledge of current hospice nursing practices.
- Respond to emergencies and conduct patient assessments.
- Assists with training, counseling, and coaching of staff.
- Support best practices with EMR and documentation.
- Practice quality communication skills: verbal, written, and electronic.
- Coordinate conflict resolution, successful peer interactions, coaching of clinical staff.
- Provide patient/family and community education with effective teaching skills exhibited.
- Participate in conferences, meetings, and in-services.
- Be flexible, organized and work effectively and professionally under stress.
- Anticipates and assists with bedside nursing, rounding, and hands on care delivery.
- Demonstrates compassion, empathy, and patience when interacting with patients, families, co-workers, and members of the public.
- Represents Circle of Life to the community in a positive manner.
- Demonstrates flexibility, versatility, and a positive attitude in integrating additional duties.
