SPECIFIC DUTIES AND POSITION REQUIREMENTS

  1. Manages the Care Navigation Program, maintains frequent communication with clients enrolled in the program, provides community education, performs informational visits about Circle of Life programs and services, and other duties as assigned. Consults clinical supervision when needed.
  2. The Care Navigation Program Social Worker may meet with clients in their homes, in facilities, in hospitals, or correspond solely via telephone.
  3. Maintains close communication with RN liaisons and coordinates visits when nursing assessments are requested or needs are identified. Performs psychosocial assessments, practices motivational interviewing skills, documents correspondence, participates in the morning pending call, maintains spreadsheets and data related to hospice conversion rates, diagnosis, and length of stay.
  4. Participates in presentations and grant proposals seeking funding for the Care Navigation Program.
  5. Utilizes the electronic medical records system on a daily basis. Frequent use of the computer and telephone communication. All Social work care will be delivered under the guidance of the best standards of practice, the NASW code of ethics, COL's policy and procedure and the Arkansas Social Work licensure board
  6. Demonstrates compassion, empathy, and patience when interacting with patients, families, co-workers, and members of the public.
  7. Represents Circle of Life to the community in a positive manner.
  8. Demonstrates flexibility, versatility and a positive attitude in integrating additional duties.
  9. Performs other duties as assigned.

Requirements: Valid Arkansas RN license current and without restriction. Access to reliable vehicle and ability to drive for extended periods of time. Valid Arkansas driver's license and Liability Insurance as required by law. Satisfactory driving record. Should be emotionally mature, objective, and sensitive to people and their problems, able to handle responsibility, work independently, and maintain good working relationships with patients, families and coworkers. Ability to speak English clearly and communicate effectively in person and by phone. Proficient computer skills required. Bilingual abilities preferred. Excellent telephone, reading, writing, and communicating skills are required. Ability to organize information, time and workload to optimize efficiency. Ability to work outside normal office hours, including on-call coverage, and to handle emergencies as required in order to meet needs of patients and families. Knowledgeable of social work ethics, best practices, community resources, and grief and bereavement services. Ability to work within an interdisciplinary team, demonstrate professional boundaries, and display a strong work ethic.
